Our office staff takes calls from 7:30am to 5pm, Monday through Friday. We do not close for lunch. After 5pm, calls are received by our answering service and forwarded to the physician on call.

Premier Cleveland Information for Patients

Before Your Visit

If this is your first visit to our office, you will need to complete our  New Patient paperwork. You may download and print the forms at our CLEVELAND PATIENT FORMS page and fill them out at your convenience before your appointment. If you have not completed your paperwork ahead of time, please arrive 20 minutes before your appointment do you may fill out your paperwork when you arrive at the office.

Medication List

Please bring a list of all your current medications (including any herbal supplements) and bring it to your appointments. This is especially important for new patients, or if you have not seen our physicians in several months.  For each appointment, please update your medication list and be sure to tell us if there has been a change in your medications or health.

Prescriptions & Refills

Please call Monday through Friday early in the day with medication requests. If you leave a message, you must state the medication name and dosage as well as your pharmacy name and phone number.  Narcotic pain medication cannot be called in – a printed prescription must be picked up during office hours. We do not handle requests for medications at night or on weekends.

Insurance & Billing

Please provide current copies of your insurance card(s) and immediately notify us of any changes. We also need your co-pay, deductible and co-insurance information. If you have no insurance, we will work with you to communicate the financial aspects of your care.

Financial Policy

Premier Surgical Associates will file your insurance for you and follow up with your insurance company(s) for sixty (60) days. After 60 days, you will be responsible for any remaining balance. You are responsible for payment regardless of your insurance coverage or legal representation in situations such as accidents or workers compensation.

Office Visits

All co-pays, deductibles or co-insurance are due at the time of service.


If surgery is required, you will be responsible for all or a portion of the fee. We will file your claim directly to your insurance company. Please remember that most insurance companies do not pay the full amount, and therefore, you are responsible for the balance. If there is a problem in paying the balance in full, please let us know and we will be happy to work with you.

Interest & Fees

Balances that remain unpaid for 30 days are subject to interest at 18% APR. Balances that are turned over to collection agencies are assessed a 30% collection fee.

Worker’s Compensation

You must contact your employer and your workers compensation insurance company and be assigned a case worker before we can treat you for a worker’s compensation injury.
