Vascular Access RN

The Vascular Access RN is first an advocate for the patient. Responsible for coordination of all needs of the patient with Acute or Chronic Renal Failure. Then primarily responsible for key relationship management including Nephrologists, Dialysis Clinic Managers and clinical staff members. The Vascular Access Coordinator should be intimately familiar with each patient’s progress from original fistula/graft to first dialysis treatments, to follow-up access maintenance and be constantly in contact with all the key stakeholders reporting patient condition following every procedure involving any interaction with Premier.
Education:
- Currently licensed to practice professional nursing in the State of Tennessee and must be familiar with the basic nursing performance standards.
- Graduate of an accredited school of nursing or Medical Assistant Program.
- A minimum of two years’ experience as a registered nurse and or CMA.
- BLS preferred
- Excellent organizational skills and verbal and/or written communication skills
- Knowledge and experience in the preoperative and postoperative care of the procedure patient.
- Knowledge of specialized equipment used in the holding/operating room.
- Experience
- Experience with hemodialysis access strongly preferred.
Job Duties and Responsibilities
- Main contact for all scheduling and coordination duties of the Vascular Access patient
- Ability to communicate and collaborate with entire dialysis access team to determine best outcomes for patients
- Coordinates all calls from dialysis clinics for procedure scheduling and any patient care concerns.
- Remains intimately knowledgeable of all patients’ condition, and needs.
- Ability to track dialysis patients to ensure follow-up appointments are made and rescheduled if missed
- Develops close relationship with dialysis clinic managers and staff nurses.
- Administrative duties to include faxing operative notes to appropriate clinics, contacting patients/clinics regarding test results, or advising patient of surgery times/location
- Performs all other duties as assigned
Operative Responsibilities

· Ensures that proper procedural techniques and practices are used.
- · Assists surgeons, and other personnel as needed to ensure outstanding patient care.
- · Records required data in EHR and updates records in the patient’s charts and tracks department statistics as requested.
- · Provides orderly progression of schedule and continuity of individual patient care.
- · Coordinates with key stakeholders to ensure optimal patient outcomes
- · Provides for psychological needs of patient.
- · Assists in establishment and maintenance of procedures, policies and standards.
- · Assists in supervision of personnel and department to ensure a safe environment.
- · Ensures clean, safe environment through continued alertness to safety and explosive hazards and aseptic techniques.
- · Assists in control of traffic and personnel to ensure proper control of infection and asepsis.
- · Assist in staff development and education
- · Reviews pertinent periodicals, scientific and medical and related literature.
- · On –call for emergencies and available for additional duty as needed.
